From form to consultation
The site handles the first step. The provider handles the rest.
- Send the form from the request page.
- We pass your details and message to a provider near Jacksonville.
- The provider gets in touch to answer first questions and book a consultation if you want one.
- At the consultation, a licensed provider decides what is suitable for you, and you choose whether to proceed.
After you send a request
Your request goes to a provider in or near Jacksonville. The provider is the one who contacts you, by phone, text or email, based on the consent you gave on the form.
When they get in touch, they may ask a few questions about what you are looking for and suggest a time for a consultation. You can accept, ask for another time, or say you are no longer interested. Nothing is booked until you agree to it with the provider.
How your request is passed on
When you send the form, your name, contact details, chosen service and message are passed to a local provider. The provider uses them to get in touch with you about a consultation.
We do not ask for health records, and you should not send them through the form. If you would like to stop being contacted or withdraw your request, the privacy page tells you how.
